What is premium relief and who can apply for it?
Premium relief is a form of state support for individuals in Switzerland whose income is insufficient to pay health insurance premiums. The scheme is administered by the cantons and aims to ensure access to basic health insurance for all. If you have relocated to Switzerland or have a low income, premium relief may be relevant for you.
In principle, any person who must be insured in Switzerland and whose income falls below a certain threshold can apply. The exact thresholds and scope of support vary by canton — some cantons reimburse up to 80% of the premium, others less. Your adviser can help you understand the cantonal regulations for your place of residence.
Income thresholds for premium relief
Whether you are eligible for premium relief depends primarily on your income and assets. The cantons set their own thresholds, which often range between CHF 15,000 and CHF 25,000 per year for individual persons — however, these figures are not uniform across the country. Important: this concerns not only earned income, but your total available income, including income from assets.
Assets also play a role — if you exceed a certain threshold (often between CHF 100,000 and CHF 150,000, depending on the canton), a portion of your assets will be credited as "income". As a newcomer, you should therefore disclose all your financial circumstances, and your adviser can advise you on which documents are required.
How do you apply for premium relief?
The application is submitted directly to your municipality or the cantonal health authority — not to the health insurance company itself. In many cantons, you can download the application form online or fill it in directly on the municipality's website. Some cantons also offer a free appointment with your adviser to prepare the application documents.
You will generally need the following documents: a copy of your passport or identity card, evidence of your income (payslips, bank statements), possibly a tenancy agreement, and evidence of assets. As a foreigner, you must also provide proof of your residence permit. Submit all documents in full, otherwise your application may be delayed.
Application deadline and validity period
Switzerland has no central application deadline — your entitlement arises at the moment you submit a valid application. This is an important difference from many other countries: you can also apply for relief retroactively if you do so within certain time limits (often up to 5 years retroactively, depending on the canton). This means that an application is worthwhile even if the relief has been due to you for some time.
The validity period is normally one calendar year (1 January to 31 December). After that, you must submit a new application or renew your existing one if your circumstances have not changed materially. The canton will inform you when a renewal application is required.
Special provisions for newcomers and foreign nationals
If you have recently moved to Switzerland and have not yet registered with a health insurance company, you must do so first — entitlement to premium relief only arises with the insurance obligation. Some cantons grant special transition provisions for newcomers in the first year. Check with the cantonal health authority to see whether your canton has such provisions.
As a person with Czech, Slovak, or other migration background, you should know: it is irrelevant where you were previously insured or how long you have been in Switzerland — what matters is solely your current income and assets, as well as your insurance obligation. Your adviser can show you which documents you need to obtain from your previous place of residence so that your application can be processed more quickly.
Increased tax advantage through premium relief
An often overlooked benefit: if you receive premium relief, you can often deduct the actually paid (reduced) premiums in your tax return, not the full premium. This results in additional tax savings. You should mention these details to your adviser so that everything is correctly recorded in your next tax return.
What to avoid: common mistakes
Many newcomers make the mistake of applying for premium relief only when they face financial hardship. The problem: entitlement only begins with your complete application. Therefore, it is better to check immediately upon your arrival whether you are eligible. Another mistake is failing to report changes in assets — this can lead to repayment demands.
Also ensure that you meet the renewal deadlines. If you do not submit a new application and you are still eligible, the support will end automatically on 31 December of the previous year.
Contact and further information
The most reliable sources for your canton-specific rules are the cantonal health authority or the municipal authority in your place of residence. Alternatively, you can search on ch.ch (the official Swiss information portal) for "premium relief + [your canton]". This will quickly give you an overview of the exact deadlines and thresholds.
If you are unsure whether you are eligible, or if the process seems complicated, arrange a free consultation appointment with your adviser. This way you can clarify any outstanding questions and ensure that your application is submitted completely and correctly.
